GRANDPARENT CARETAKER AFFIDAVIT
GRANDPARENT POWER OF ATTORNEY

Beginning in July, 2004, Grandparents may file paperwork to have physical custody of their grandchildren. The paperwork assists grandparents in enrolling the grandchildren in school and receiving medical treatment. IT IS NOT LEGAL CUSTODY.

Juvenile Court Staff cannot assist in completing the paperwork, so please contact the Kinship Caregiver Coalition and schedule an appointment, at 937-775-8245 (Intake), 937-775-8246 and 937-775-8247 (Kinship Navigators).
