1. Print the General Information and Directions, Personal Information Sheet, and Custody, Visitation, Support Packet
2. Read:  General Information and Directions
3. Contact Dayton Bar Association / Legal Aid for any help needed
4. Take completed Personal Information Sheet, pro se packet(s) to Juvenile Court – Clerk’s Office
5. Follow all other instructions on General Information and Directions
